When adding files to books, there are two Add Files to Book dialog boxes. If you press Alt+B or click browse in the first one, the second one (which is really a standard windows open dialog box with the title Add Files to Book) appears.
After you select your file and press Alt+A or click add, when you are returned to the first one, for some reason the book document is active instead of the first Add Files to Book dialog box. If you are in the habit of using your mouse, this is not an issue, however, if you are a keyboard maniac such as myself, when you attempt to press Alt+A to add the book, it will not work. You must click Add.
